Travel Information

We have dedicated times allocated for our travel clinic on several days of the week, by appointment only. This clinic is run by travel health nurse practitioners who have had training in foreign travel issues. For this reason we must endeavour to use the allocated clinic times only.

Travel questionnaires are available to collect at the main Practice reception, and branch surgeries reception, or downloaded from Practice website. This must be handed back within at least 6 weeks before you intend to travel, due to the time necessary to assess which immunisations are required, suitability for immunisation, arranging prescriptions, ordering of vaccines and allocation of appointments. We do not accept any forms within 6 weeks of departure date. In these cases you are advised to contact a private travel clinic.

Please complete your travel questionnaire, adding any other information which you feel may be relevant for your travel. Then hand the form to the receptionist who will pass it to the practice nurse for assessment of immunisations recommended and required. The practice nurse will contact you by telephone, or letter, within 3 weeks to arrange appointments for immunisations, some of which there may be a charge (where applicable, payments are required prior to administering immunisations), and discuss travel health risks and advice.

Further information may be accessed at: www.fitfortravel.nhs.uk and www.fco.gov.uk/en/travel-and-living
